- Read about the survey and reward and click next.
- Read about how to start the survey and view the sample receipt. Enter the serious number (16 digits) and the store number from the receipt.
- Enter the date of your visit, how many times you have visited Donatos, the time of day you ordered, how you received the order and how you placed the order. Also, select how long you waited to get your order.
- Rate the delivery driver or the order taker, depending on how you ordered.
- Rate the statements provided using the scale.
- Rate the value of the price, whether you will recommend and return to the restaurant. Also, answer a few yes or no questions about your food and any problems. If you did have problems, a text box will appear on the next page asking you to include extra information about your problem. This is optional, but can be very helpful to the company. Please consider explaining your problem and how it was resolved, if applicable.
- Read the thank you and receive your code validating the offer. Write this code on your receipt and take it to the store to redeem. You can do this every two weeks to receive a free pizza.
